This notice explains how information submitted through H&G Advanced Skin Institute forms is used for the specific request selected by the visitor.
Effective September 17, 2026 · Version 2026-09-17-1
Effective date. This notice is effective September 17, 2026. It describes the Institute’s current handling of information submitted through this website and is not a claim of compliance with any particular law.
Organization. H&G Advanced Skin Institute is a DBA of H & Glow LLC. Questions about this notice or a submitted request may be sent to skininstitute.hng@gmail.com.
Information collected. Depending on the form, the Institute may collect contact information, location, professional role, organization or school, license information, experience, areas of expertise, proposed educational topics, professional links, workshop selections, accessibility requests, and other information the visitor chooses to provide. Visitors should not submit medical records, patient information, Social Security numbers, identity documents, banking information, or other unnecessary sensitive information.
Purpose. Information is used only to review and respond to the selected inquiry, expert application, or course and workshop registration. An expert application is reviewed manually and does not create employment, appointment, affiliation, a public profile, or permission to use Institute titles. A registration request does not confirm eligibility, availability, attendance, continuing education credit, or access to paid materials.
Form service. Online forms are intended to be transmitted to Formspree, a third-party form delivery provider, and forwarded to the Institute’s designated email account. Formspree processes the submitted fields to provide form delivery, spam controls, and account history according to its own terms and privacy practices. The Institute does not place passwords, private management tokens, or payment information in these forms.
Retention and manual records. Formspree account history is limited by the account plan and settings. The Institute may retain submission emails and manually transfer necessary fields into a private administrative record for follow-up, eligibility review, registration administration, and recordkeeping. Information should be kept only for as long as reasonably needed for those purposes and applicable obligations; this notice does not establish a guaranteed deletion schedule.
Sharing and publication. Form data is not published automatically. Applicant biographies, license details, and professional links are not displayed merely because they were submitted. Information may be reviewed by authorized personnel or professional reviewers only as needed for the request. The Institute does not treat consent to process a request as consent to receive marketing messages.
Accuracy, security, and choices. Visitors may choose not to submit a form and may contact the Institute by email instead. No internet transmission or email system can be guaranteed completely secure. To ask about, correct, or request deletion of information held by the Institute, contact the email address above; the response may depend on the Institute’s legal and recordkeeping obligations.
Changes. The Institute may revise this notice as its forms, providers, or administrative practices change. The version shown with a submission is included in the form record for reference.
